AUTHORIZATION TO DELIVER AND SERVICE WITHOUT A SIGNATURE, AGREEMENT TO PAYMENT TERMS, AND TERMS AND CONDITIONS.
The undersigned authorize Peaks Island Fuel to deliver fuel and /or service of fuel related equipment without a signature and will be responsible for payment of services in accordance with Peaks Island Fuel terms of payment. In the event this account is placed with a third party for collection, the undersigned agree to pay for all costs, including reasonable attorney fees, court costs, and finance charges/late payment fees. The undersigned also acknowledge that service may be disconnected and/or discontinued if payment is not made in accordance with Peaks Island Fuel terms of payment. This authorization for delivery of fuel and/or service of fuel related equipment will remain in effect until terminated by the undersigned by means of 30 days written notice to Peaks Island Fuel. All propane storage equipment including but not limited to propane tanks, regulating equipment, meters, and foundation clock will remain exclusive property of Peaks Island Fuel unless otherwise stated. The undersigned also agrees to pay for all charges associated with the removal and additions of fuel storage and fuel supply equipment if applicable. I have read and agree to all the terms and conditions of this agreement as set forth on the front and following pages.

Peaks Island Fuel Terms and Conditions
FUEL PRICING: All fuel pricing (Propane, #2 Fuel Oil, and Kerosene) is subject to market fluctuation and to change without notice.
PROPANE EQUIPMENT: Peaks Island Fuel currently provides their Peaks Island customers with propane bottles free of charge. Homeowners will be notified if the propane tank at their location is in need of replacement or gas service is in need of inspection to ensure quality and safety of service. Starting this summer, if bottles need to be swapped out, there will be a setting fee and the homeowners are responsible for the cost of the regulator and minimal shop supplies to do so. This charge is necessary due to the ever-rising cost of doing business in the fluctuating market of this industry. Peaks Island Fuel will be putting a minimum usage cap for people eligible for free propane tank use. Peaks Island Fuel has to get at least 4 deliveries a year in order to qualify for a free tank use. If the customer does not qualify, they are responsible for buying their own tank once the tank is in need of being swapped out.
AUTOMATIC DELIVERY: Customer authorizes Peaks Island Fuel to deliver fuel on an automatic basis as determined by Peaks Island Fuel using degree-days, Julian calendar, historic consumption, and/or estimated consumption. Peaks Island Fuel assumes the responsibility to ensure the customer does not run out of fuel as long as the customer’s account is in good standing and the customer’s usage doesn’t deviate from historical usage or estimated usage. Customer agrees to limit liability of Peaks Island Fuel with respect to fuel outages to all costs associated with delivery of fuel by Peaks Island Fuel and priming oil system and/or pressure testing propane system. Customer agrees to hold harmless Peaks Island Fuel for any and all fuel outages associated with “Force Majeure” situations and/or the customer’s account being in poor standing.
WILL CALL DELIVERY: Customer authorizes Peaks Island Fuel to deliver fuel when ordered by the customer. Customer assumes complete responsibility to order fuel in a timely manner as needed. Peaks Island Fuel requires 48 hours notice before a delivery is made and that the account is in good standing. If customer wants and/or needs fuel delivered before next scheduled trip or before 48 hours (i.e. special trip) customer agrees to pay for fuel delivery as well as any and all special trip charges associated with the delivery. Customers agree to hold harmless Peaks Island Fuel for any and all costs associated with fuel outages or inability to deliver in a timely manner.
FORMS OF PAYMENT: Cash, Money Order, Check, Credit Card, and Debit Card (Mastercard, Visa, Discover only).
COD ACCOUNT: Peaks Island Fuel will deliver fuel and provide service to customers on a COD basis when the customer is not approved for credit or requests to be a COD account. Peaks Island Fuel requires the following information on our customer agreement for a COD Account: Customer name, Billing/Mailing Address, Home Phone, Work Phone, and Drivers License Number and State. This information is used to establish and set up your account and is required for pre-authorization payment by check, credit card or debit card as well as third party collection in the event the account is turned over to a third party for collection. This information will not be used to obtain credit reports or check credit viability when the customer requests to be on a COD account. A COD account must pay for all deliveries and/or service upon receipt of delivery and/or service. Payment must be made in full in the form of cash, money order, bank check, or if approved by pre-authorized credit card or debit card. Any charges not paid in full are subject to the same terms and conditions as a credit account. COD accounts are also subject to same terms and conditions as automatic delivery and/or will call delivery where applicable.
PRE-AUTHORIZED PAYMENT: Peaks Island Fuel will at its discretion offer pre-authorized payment by credit card or debit card. Customer must fill out, sign, and date Peaks Island Fuel Credit Card-EFT Authorization form. Accounts utilizing pre-authorized payment are also subject to the same terms and conditions as automatic delivery and/or will-call as applicable. It is customer’s responsibility to notify Peaks Island Fuel to update new credit card information (i.e. new expiration dates, number, zip code, or CVC number). If credit card on file is declined and Peaks Island Fuel is unable to collect payment, any scheduled services or automatic deliveries will be put on hold until a valid credit card is given or previous service and/or delivery is paid for in full.
Peaks Island Fuel has the right to change the status or type of an account at any time.
HOMEOWNER’S RESPONSIBILITES AND LABOR FEES

For Will-Call accounts, we request 48 hours before the scheduled delivery. Please keep track of your tank gauges to ensure you give yourself time for a delivery before your fuel runs out. If you do run out and we make a special trip delivery, there will be a $50 fee during normal business hours and $100 for nights (after 4.30 PM) and weekends (Saturdays, Sundays, and holidays). If the homeowner needs pilots lit, furnaces restarted, or lines bled, then there is an additional $50 fee.
For customers who need service work done, standard labor charges are $125 per hour (Monday thru Friday 8 AM-4.30 PM). After Hours (after 4.30 PM, weekends and holidays) emergency service is $187.50 per hour.
For those customer’s receiving deliveries and service through the winter months (whether automatic delivery or will-call delivery), we ask that all pathways and driveways to tanks/bottles, basement doors and bulkheads are shoveled properly to ensure that our drivers and technicians are able to safely and efficiently make your delivery. Please keep in mind that the fuel reels on our fuel trucks are extremely heavy and it can be very dangerous and extremely labor intensive to pull them through snow banks, ice and snow. For those people who live elsewhere in the winter and keep Peaks Island homes open through the winter, it is your responsibility to set up an account with someone who can plow and shovel so we are able to continue delivering. If we cannot get to the house and fill pipes, we will not deliver. If you are on automatic delivery, Peaks Island Fuel is not liable for fuel outage due to unshoveled or unplowed driveways and paths.
For those customers that rent their houses and the renters are responsible for fuel, it is required that the renters call us to establish an account and have a credit card on file.
